Program Access and Eligibility
Referral Requirements
Entry into the program requires a referral either from a client's health plan or directly from the client themselves. This ensures that all participants are appropriately placed to benefit from the program’s offerings.
Automatic Client Transition
Deblin clients transitioning from inpatient care are automatically eligible to participate in our Day Program for Acute Needs, facilitating a seamless continuation of support.
Transportation Support
Transportation to the program is coordinated by our Acute Day staff in conjunction with a client's assigned Qualified Mental Health Professional (QMHP), ensuring reliable and stress-free access to our services.
Targeted Clientele
This program is inclusive for clients who are already engaged in Intensive Case Management with us. It is not open to clients receiving lower levels of care or those enrolled in children and family services. This focus allows us to tailor our resources and expertise to the specific needs of our most acute cases.
