Timely Follow-Up 

A Qualified Mental Health Professional (QMHP) or Licensed Mental Health Professional (LMHP) will meet with the client on the day of discharge or within 8 hours of inpatient discharge. This ensures immediate support during the critical initial hours post-discharge. 

Comprehensive Review and Assessment 

The professional will review the hospital’s discharge plan with the client to ensure understanding and feasibility. They will also conduct evaluations and a risk assessment to determine the client’s stability and readiness to continue recovery in a community setting. 

Barrier Identification and Intervention 

We focus on identifying and addressing any barriers that might hinder the client’s ability to thrive in a barrier-free, community-based environment. Whether it’s securing medications, confirming follow-up appointments, or other necessary interventions, our team is equipped to facilitate smooth transitions. 

Medication Management 

Medications are reviewed, and arrangements are made to obtain any necessary prescriptions if not already provided, ensuring the client has the resources needed for continued health and wellness.
